Immunization records are required for all Kindergarten and 8th grade scholars.

Medication Authorization Forms are available for both prescription and over-the-counter medications. If your child takes a prescription medication during school hours, the proper documentation must be provided (a medication authorization form signed by your child’s physician). If you need a copy of the form, please reach out to the office. Scholars are not permitted to carry medication to school. Parents must bring the medication and sign it in.

If your scholar has been sick, they can return to school once they are fever-free, vomit-free, and diarrhea-free for 24 hours without the use of medication.

If your scholar has been absent, we will accept a handwritten note for the first two days of absence and will need a doctor’s note for the third day and beyond. Excused absence reasons are outlined in our handbook. Please send in any notes with your scholars within three days of their return to school.
